Insurance 277 Codes

Providers may see a 277 code filled in under the column "e277 Information" in the billing history of a claim.  277 Codes are split into three parts: Category code, Status code, and Entity code. 

The category code will indicate if a claim has been received or acknowledged by an insurance company, and may include information on whether the claim has been accepted or rejected for adjudication.  CLICK HERE for a PDF download of a full list of e277 Category codes.

The status code will indicate more information about the category code listed on the claim.  This may include information as to why a claim was rejected for adjudication.  Some status codes may be listed as "Free form text message" - this text will be available in the billing history of a claim.  CLICK HERE for a PDF download of a full list of e277 Status codes.

The entity code will indicate the entity referred to by a status code.  Often this entity will be the patient or the provider.  Some status codes require the use of an entity code, however, insurance companies will not always include this information in the 277 response file sent to the IFA.  CLICK HERE for a PDF download of a list of e277 Entity codes.
